Apple Store iPod event sweepstakes

Does anybody have a scan of the sweepstakes entry form from the iPod event (for the speakers)? I need the entire thing, including both sides. Any help would be greatly appreciated.

    I have the smaller one that they were giving out earlier that day, but I think I have an extra of the one that had the sweepstakes tab on it (with it still attatched), But if I do, it's in the bedroom and my girlfriend's asleep and I'd rather not wake her up. If I find it tomorrow morning I'll post it for ya. Seeing you're from Buffalo I'm guessing you went to Walden Galleria, me too.

    Sorry to say, all she had in there was another with the form already ripped off.
