Is there a OS record of password access?
1) Is there a file that records password queries and access attempts, with dates, times and locations, like external IPs?
2) If #1 exists, do they only store the password accounts, but not the actual passwords--real or incorrect--on the file?
3) is it true that there's no way to recover a lost master password for file vault?

No. There used be a long winded work around by using the install DVD in versions before Tiger, but I don't think there is a recovery mechanism in Tiger.
