Accessibility: Zoom screen without keyboard modifier?

I'm trying to help a user who only has the use of one hand, and he would like to be able to zoom the screen using only the mouse wheel. In the Universal Access preference, there is the option to zoom the screen with the scroll wheel using a modifier key. I can change the key, but there doesn't seem to be an option to just zoom using only the wheel.

Yes, I'm aware this will remove the ability to scroll windows with the wheel.

Can anyone think of any clever way to achieve this, or know of any third party software that will do the same? His physical therapist was apparently doing this, but I didn't get the details on the OS or other software he was using (and I'm willing to bet it was Windows).

The built in zoom functionality works great - I'm just looking for a solution that doesn't require the modifier key. Any suggestions?
