Initio Miles Ultra 2 and OS X

I could really use a hand here and by the sounds of it you guys are the pros.
I have a G4/867 Quicksilver and have been trying for the past couple of days to put an Initio Miles Ultra 2 card in it and connect my Burly enclosure that contains 2 Cudas and 2 Cheetahs. I have flashed the Miles card per the instructions on Initios web site with the Beta 1.06 sim in OS 9.2.2 without the external enclosure connected and everything worked fine; card showed up and all of the drives. Booted into OS X and the card shows up but no drives. I tried to launch the Miles_2_Smart_SCSI_X 1.00 utility and it says it can't find the driver. Please help walk me through this process and point out the obvious thing I am missing. Of course it has to be obvious
