Well, first I replaced my car's stock radio/CD player with an aftermarket unit from Kenwood. I did this anyway, as it is far better than stock. To bypass FM modulation with a CD changer you usually need a new unit anyway.
When I had it installed (see your local car stereo dealer) I had them install the RCA jacks in the AUX input of the Kenwood, and then ran RCA cables (like on a home stereo) into the car's cabin. There I have a 2 RCA-to-minijack Y-adaptor like this one:

And then this plugs into my iPod, which is held into place by a slightly-modified (custom) cell-phone cradle.
Awesome setup. Beats the HELL out of a radio RM transmitter (iRock, etc) and ownz anything like a tape adaptor.
