Headphones cause spinning wheel of death

Whenever headphones are used with my mom's macbook pro, she has to restart the computer due to the spinning wheel of death.  She has tried several different brands/kinds, but nothing seems to help. Anyone else have the same problem and also has a solution?

Macbook pro 2013
os 10.9.5

    Maybe an irrelevant question, but what happens if you try to plug in an external speaker instead? Preferably one that is as “dumb” as possible–something that is designed to just reproduce sound, without controls that feed back through the system into which they’re plugged in.
