Why do I keep getting podcast episodes I have already played?

I listen to podcasts on my walks, using iPhone, Apple Watch, & AirPods.   Quite often when I start playing, it plays podcast episodes I have already listened to.   I look at the options for that podcast and don't see any options to remove that podcast.   I fast forward through them and often get an older episode of that podcast (or two or three).   This doesn't happen on my Mac.

This happens maybe once per week or so.

How can I stop this?
