Certain video cards support pivoting on the Mac (for instance, the retail Radeon 9800 Pro). ATI's marketing name for this ability is Versavision. I've been told that some nVidia cards support this feature as well, but I don't know which ones or what they call it.
EDIT: I should add that I don't know if any of these cards will automatically detect the rotation of the monitor, so if you change orientations often, you might want to make a script or something.
