I think you may be confused as to what sendmail is. All it does is transfer mail in and out of the mail queue on your system, if you are using it. It is a mail server.
No Mail is not "built on top" of it. Mail can USE sendmail to send email from your system. If you choose to run sendmail and you want Mail to check the email in your message queue for you, then you have to set up a Unix account. I think that's what they call it in there, it may also be a local account.
