Blue screen of death

Twice in the last two days, we've logged out (out of different accounts) to find that the login screen (with the list of accounts) was gone. All we had was a blue screen. Once the synthesised voice announced "caps lock" eveytime I pressed a key. I've had to do the dreaded manual reboot twice.



I am running 10.4.7 on a PPC G5.



What's going on? Is there a key combination (like Ctrl-Alt-Backspace in Linux) which would take me out of the desktop and into a console enviroment which can allow me to safely reboot?

    I think holding down SHIFT while booting loads OS X up in safe mode.
