Problem with volume control on Intel Mac Mini

I can't seem to adjust volume any more on my Intel Mac mini using the apple keyboard or the IR remote. I could see volume icon pop up when I hit the volume button and it also moves up and down as I want it to, but the volume it self stays the same. Only way I could adjust the volume now is through the volume in menu bar. I appreciate any help to fix this.

    If you're using the optical (SPDIF) out, you can't change the output levels. Or are you referring to the internal speaker?

    I'm using the internal speakers and I use headphones sometimes, I haven't changed anything just a couple of days ago this problem started.
