Text Captions in MS Word

I've created an MS Word 2008 legal document where in the paragraph a person would fill in their name and other information. What I would like to do is have small text underneath the underline that would basically describe the field, like a caption. I'm able to make text subscripted, but cannot get the text to be underneath the line. I'm sure that this can be done but my Google search proved fruitless. Any info would be much appreciated!

    Legal document? My advice to you is that if you don't know how to do a legal document then you should hire someone who does. Seriously.

    You are using the wrong tool for the job. What you want is a PAGE LAYOUT program like InDesign.

    Nobody said that you were an attorney. It was obvious from your post that your friend is the attorney. It is your friend who can't be much of an attorney because you came to us looking for help with a legal issue rather than going to him.
