I find having the keyboard, tablet, mouse lower than the monitor's position is the best for me.
I can't have this "holy trinity" (k,t,m) at the same height level as the monitor. That would mean the monitor will be placed lower. This will hurt my neck. If I adjust the my chair lower to fit my eye level with the monitor that would mean I will be hurting my elbows (the holy trinity is higher)
There are plenty for 17", and a few that'll do for 19".
You can still get a telescoping keyboard arm to bolt to the underside of the desk. Or, you can get a nice small footprint monitor shelf made up. Three pieces of wood will do. I'm pretty sure you can find some to buy ready made too. Then you can even use the underside of the little shelf for your modem, a USB/firewire hub, and external CDRW (all tucked neatly under the monitor.
Another nice alternative is a shelf that attaches to the back edge of your desk. Ikea sells units that are about a foot deep and can run the length of your desk. Then you still have a completely open surface in front of you for papers and the like, and you can prop the monitor up higher while tucking less used extras under the shelf.

Today was Mac day at our house, got a new Dual with the ACD and my wife got a new Ti for work. Everything has been pretty good so far, I've got a few software kinks to work out but damn this thing is fast. The monitor is pretty unbelievable - It's going to take a few days to get used to all of this real estate.

You know, the second after I posted I looked at your picture again, remembered the B&W 17"ers we have in the lab, and thought of exactly that problem. Did anyone ever make a replacement tripod for the Studio CRTs that was taller? or perhaps the disc style? That's a toughie. I wonder if the tripod base attaches to a standard pivot of some kind that can accept a disc type base from another monitor. You could always paint it so it wouldn't look like crap.
Hopefully, in the not too distant future, all monitors will be LCD displays on iMac style adjustable arms.

IKEA desks are almost always (not always) ripoffs of much more expensive designer furniture. I've seen Corbu, Eames and Eileen Gray ripoffs regularly in their catalogs. Not exact duplicates by any means, more like those later iMac wannabe PCs. Take a look at IKEA stuff and then look at the originals (and their outrageous prices!):

you reminded me of a bit of photoshopping my pal rich did... he designed a custom orange imac with the longhorns logo on the side... fer some japmac magazine cover...
i went to his site... but he's taken them down already. still there's some nifty items to be had.

I can reach behind myself easily and get a CD or whatever from a bookshelf that is only a couple of feet from the desk. I also have a window behind the desk, so any high shelves would ruin that view...
