Stomach Pouch

I have two people at work that got their stomachs pouched because they couldn't stop stuffing food in their faces.



The first one got it done coupla years ago and went from obese to what could be called thin (at least for where I work where all the women seem to be rather fat).



The second one just got it done. She's much younger and not as obese. Rumor is she had to stop around for a surgeon willing to do it. We see what she eats at work and she's never been on any kind of diet. Maybe the see food diet?



I'm surfing Yahoo groups and see adds for "bariatric surgery" which I guess is the stomach pouch. Is this the new boob job/face lift? Maybe there will be a reality show out of it?

    Maybe we need to start a North Korea/U.S. citizen exchange program. Too fat? Go eat hay and pebbles in North Korea.



    Hard to feel sorry for overstuffed Americans, when most people die from having no food at all.



    Must be nice to be a $urgeon in the U.S., though.

    They could also have tried the "4£ diet"? Eat anything you like, and as much as you like. But whatever and everything you eat in one week may not cost more than 4 uk£. That is pretty effective, I by mistake found out while studying in UK with no grant.



    My friends have also tried "Ireland diet", with very good success. Get cheap tickets to Ireland, and .. drink all the guinness you can and want. And once they came back they were visibly thinner than before they went to Ireland.

    I don't practice stomach pouch surgery, but as a plastic surgeon, I practice surgery on this patients after weigh loss in a regular way.



    Normally_bariatic surgery is for people with an body index superior of 35 (40 is better) who did not succeed well conducted diet (and not a simple try), who have a good mental health (it's worthless on psychiatric cases), and who have been checked by an endocrinologist to see if nothing is wrong in this aera.



    Bariatic surgery leads to gain of survey if practiced under laparoscopia, otherwise the post surgery mortality is superior to the life expectancy gain and thus worthless. This is a dangerous surgery and especially_banding, can lead to troubles. There is many trials, with banding, when the surgeon did not operate soon enough, early_complications, like stomach perforation (typically with banding, the patient want to leave hospital three days later, otherwise, it's better to do an another laparoscopic check). There is also various techniques like vertical reduction, who have others cons and advantages.



    I have see many good results of bariatic surgery, with typical weight loss of more than 60 pounds, with records of 170 pounds (two case in my practice). Unfortunately for this people, if their qualitie of life is much nicier, the esthetic result are horrible, their skin tend to flatter like a flag in no-wind condition. That's where I work, but if I can have results, it's far from perfect.

    People (who have been selected carefully) tend to be very happy of this surgery



    My advice is : it's a good surgery, but a serious one, needed to be practiced by a specialised professional, with serious skills in this particular aera, associated with a professional team, to select the good cases. Before such surgery, people should seriously consider a real diet, that will be always better than this. This surgery have his place only in case of failure of such diets. This people tend to still have hunger, but cannot eat too much, otherwise they will be punish automatically (they_want to vomit if they eat too much).

    A question Doc. Did the skin just lose the elasticity because the people were so large they basically overstretched it, or was the factor that made it that way the rapid loss of weight due to not being able to really eat much? I guess what I am getting at is if these carefully screened folks had been able to lose the weight via diet and exercise, would the skin still have been baggy or would it have shrunk back to normap proportions?

    Unlike reptiles, mammal's skin (and especially human skin) has the property to grow with the individual. Unfortunately the reverse is not true. Skin growth is induced by an excessive skin tension (that how work skin expansion : you put a baloon under the skin and inflate it, in order to gain skin). The amount of stretch, a skin is able to support depends of the natural elasticity of the skin. The more, the skin is elastic, the more tension it can support, before growing.

    Elasticity of the skin depands of the age, the way of life (sun and tabacco are wrong), hormones and the genes.



    The obese people who have losen weight, have to much skin, and the natural elasticity of the skin is not always (read rarely) sufficiant to obtain the necessary tension. Without the necessary tension the skin fall apart. The less the skin is elastic the worse it will be.



    I don't think that the speed of weigthing loss, is important for this issue, but the reverse is certainly true. Huge weight gain, in conjonction of a increase of hormones, can lead to overstreching (read vergetures). Vergetures are a particular type of scar, representing the tearing of the Elastine who is broken. Such skins are a disaster in a esthetic point of vue. They are vergetured, but they have also no elasticity : it means that this type of skin will always looks like unbended. That's why the best advice I can give to a pregnant woman, is to not taking weight too quickly, especially in the last months of the pregnancy.



    The best way to preserve skin is to not take too much weight, it's a very cheap and silly advice, but it's certainly true. Sometimes when I see some patients who have loose 170 pounds, the result after my surgery is awfull (only good for wearing clothes), but the result before surgery is nightmarish, and will have their places in gore movies.

    Pouch Girl was back at work today for a party. She didn't eat anything. Still on a liquid diet. Better than the see food diet she was on.



    She looked okay. Not any thinner but not on death's door step either.

    My friend from Romania used to say all the time, "Could you borrow me a dollar" or "Can I lend a dollar from you?" They have one word for both borrow and lend and he could never keep them straight in english.
