AT&T and Verizon signed a cross-carrier VoLTE agreement in 2014 that was supposed to start in 2015. I guess AT&T got it working just in time to meet the 2015 promise.

Lightning won't become an industry standard because it's proprietary to Apple, but perhaps this would jumpstart the Bluetooth headset market. Hopefully Apple can use the freed-up space to reduce the bezel size.

This seems doubtful. They just signed a bunch of people onto the iPhone Update Program, which allows for upgrades after 12 months. That plan is popular with people who upgrade every year.
