Seriously, your line of "never gotten livestreams right" is bullshit. The past few years streams have been generally fine, except this time. In terms of pre-orders, point me to another company on the planet that successfully processes millions of online pre-orders within minutes? Oh wait, noone else on the planet gets those kinds of numbers so its an impossible comparison to make.

I just checked mine and same here I managed to order mine using the apple website on my iPhone while stuck in traffic on my drive into work this morning, which made going nowhere for 15 minutes a bit more enjoyable in the end.

Probably not a charge, but a pre-authorization. They want to make sure you have the money. They complete the pre-authorization when they ship. This is how it worked in the past. If they never complete the pre-authorization (or don't do it in time) then it will expire and disappear without ever showing up on your bill.
Because the orders have to connect to the carriers systems to validate the older phone being upgraded, they have to check that the customers current phone is past its 2 year anniversary and eligible for upgrade. So its not as simple as sending back flashy website graphics. most orders are for replacing an older phone the market is pretty much saturated in the USA
But this is 2014 and in the big picture, even the huge number of iPhone customers is no match to technology.
Unless prohibited by law, Apple could negotiate a customer upgrade eligibility status and option dump from each carrier in the USA ahead of the launch and do preliminary processing offline (like they do with CC numbers). Even if there are a billion potential customers, that data file would by a few GB, hardly a technological hurdle.
Personally, I think any technical justification (not explanation) of a ordering bottleneck nowadays is laughable.
The thing is, it doesn't matter if Apple can handle it, if the cell companies can not.
When you order a Verizon phone from Apple, Apple has to communicate with Verizon, isn't that how it works?
Clearly this is the real issue and the only issue. When Apple is handling this all alone generally such issues don't occur. The only time (for me) Ive had issues is when the purchasing process includes their servers "attempting" to talk to AT&T's (in my case).

Is that reality? No company on Earth delivers or even has attempted to deliver fully on demand on ANY product in recent memory. Name one that equals something similar.
The fact that EVERY iPhone transaction is subject to gaining access to third party servers and the challenges that come with that - I'd say cut them a break. You can't hold Apple liable for not being able to secure data, approvals, etc etc etc from a third party, when, IN FACT, the third party themselves is caving in because of demand.
Maybe the headline should be... Since AT&T takes in ALL OF MY CONNECTIVITY MONEY -- how bout after ALL THESE YEARS - they FIX they're own system to make buying product from Apple easier on IT'S customers.
Part of me thinks that the Telecom's collectively stifled the process last night in order to gain more volume on their own sites keeping their customers in the family so to speak. I mean they all have a vested interest to make it hard to buy the phones via Apples portals versus driving traffic unto their site and into their own stores. This feels more like it.
But relying on data approvals from 4 or 5 different Telecom companies is a nightmare no matter who is selling the product..

I kind of felt that way about the stream. The last two or three events have been streamed with no issues at all. Then there is a failure but only on the website. The stream worked perfectly on Apple TVs. Maybe They should have everyone to sign in via their Apple ID to see the stream.
I think the issue with this stream was that they weren't at the Moscone Center where they've held the previous recent events and refined the broadcast capabilities. As soon as I saw they were doing this from the Flint Center, I had a feeling the stream would go up in smoke. Once Apple Campus 2 is done and they have their own special event center, they won't have an excuse for proper piping. But for now, this last stream, I'd put on the event location's capabilities.

Avoided the whole Apple website fiasco by ordering mine through AT&T. Easy peasy...

This morning around 9am, I ordered black iPhone with 64GB and it went through without a glitch. And yes it will be ready on 19th.
